A 58-year-old woman notices that the whites of her eyes have turned the color of old parchment while rinsing a coffee cup. Her urine has been dark for a week, her stools “looked almost gray” yesterday, and a dull ache has settled beneath her right ribs. She is afebrile but uncomfortable, with a pulse of 104 and a faint yellow cast to her skin. The next move—deciding whether this is obstruction, hepatocellular injury, or hemolysis—has not yet been made.

When to Think of It

Think jaundice with yellow sclera/skin, dark urine, pale stools, pruritus, abdominal pain, fever, encephalopathy, bleeding, or unexplained constitutional symptoms. Ask about timing, alcohol, medications/supplements, viral exposures, transfusion, pregnancy, travel, prior liver disease, and hemolysis.

Sick or Not Sick

Sick vs. not sick: the key call is whether the patient has sepsis from biliary obstruction or acute liver failure—fever/hypotension, altered mentation, coagulopathy, hypoglycemia, severe pain, or rising lactate mandates resuscitation and urgent specialty involvement.

The First Fifteen Minutes

  • Obtain two IVs, cardiac monitoring, glucose, CBC, CMP with AST/ALT/alkaline phosphatase, fractionated bilirubin, PT/INR, lipase, urinalysis, pregnancy test when relevant, and type and screen.
  • Glucose <60 mg/dL or altered patient unable to take oral glucose → dextrose 25 g IV (50 mL D50W; use 100 mL D10W when peripheral extravasation risk is a concern), because immediate substrate prevents hypoglycemic brain injury.
  • Hypotension or sepsis with suspected cholangitis → crystalloid 1–2 L IV initially (balanced crystalloid preferred), because restoring preload supports perfusion while source control is arranged.
  • Persistent MAP <65 mmHg after fluids → norepinephrine 0.05–0.1 mcg/kg/min IV infusion, titrate, because α-mediated vasoconstriction restores organ perfusion; use a reliable dosing reference/institutional protocol for titration.
  • Suspected acute bacterial cholangitis or biliary sepsis → ceftriaxone 2 g IV plus metronidazole 500 mg IV, because this covers enteric gram-negative organisms and anaerobes pending drainage. For severe illness, healthcare-associated infection, or local resistance concerns, use piperacillin-tazobactam 4.5 g IV instead; verify renal adjustment and institutional antibiogram.
  • Suspected acetaminophen toxicity or acute liver failure with possible acetaminophen exposure → N-acetylcysteine 150 mg/kg IV over 1 hour, then 50 mg/kg over 4 hours, then 100 mg/kg over 16 hours, because it restores hepatic glutathione and may benefit non-acetaminophen acute liver failure; confirm protocol because dosing regimens vary.
  • Avoid empiric vitamin K unless deficiency/warfarin effect is plausible; INR elevation from hepatic failure will not reliably correct with vitamin K.

Definitive Care & Disposition

Conjugated hyperbilirubinemia with ductal dilation requires gastroenterology consultation and MRCP/ERCP planning; cholangitis requires urgent biliary drainage plus antibiotics. Acute liver failure—INR ≥1.5 plus encephalopathy without preexisting cirrhosis—needs ICU and transplant-center discussion. Hemolysis requires treatment of the underlying cause, not biliary imaging alone. Stable, mild isolated abnormalities may receive expedited outpatient hepatology follow-up only after dangerous causes are excluded.

How This One Kills

The lethal miss is treating “jaundice” as a lab abnormality while an obstructed, infected biliary tree progresses to septic shock—or failing to recognize early hepatic encephalopathy and coagulopathy as acute liver failure.
The Differential — What Else Looks Like This
  • Carotenemia — yellow skin with spared sclera and normal bilirubin; confusing it with jaundice triggers unnecessary invasive workup.
  • Hemolysis — indirect bilirubin, high LDH/reticulocytes, low haptoglobin; confusing it with obstruction delays treatment of anemia or hemolytic crisis.
  • Drug-induced liver injury — temporal medication/supplement exposure with variable biochemical pattern; missing it perpetuates the offending agent.
  • Acute viral hepatitis — markedly elevated aminotransferases often with prodrome; confusing it with cholangitis can delay needed drainage, while assuming it is benign can miss liver failure.

The Second-Day Story

Older adults may have little pain or fever despite serious obstruction, and immunocompromised patients may be normothermic. Acute liver failure may present first as sleepiness, irritability, nausea, or unexplained hypoglycemia before obvious jaundice. Follow the pattern—direct bilirubin, AST/ALT, alkaline phosphatase, INR, glucose, and mental status—rather than the visual intensity of the jaundice.

She is risk-stratified as sick because infection plus obstruction requires resuscitation and source control, so she receives IV crystalloid, ceftriaxone plus metronidazole, and norepinephrine if hypotension persists. Gastroenterology performs urgent ERCP for drainage, and she is admitted to the ICU for vasopressor monitoring and sepsis care.

She has no acetaminophen overdose, significant alcohol use, or encephalopathy, but is tachycardic with borderline blood pressure and has jaundice and right-upper-quadrant tenderness. Labs show predominantly direct hyperbilirubinemia, marked alkaline-phosphatase elevation, leukocytosis, and preserved glucose with an elevated but not profoundly abnormal INR. Ultrasound demonstrates a common bile duct stone with ductal dilation. My assessment is acute cholangitis from obstructive choledocholithiasis; I’m starting IV fluids and broad enteric/anaerobic antibiotics, monitoring perfusion, consulting GI for urgent ERCP, and admitting her to the ICU.”
