The First Fifteen Minutes
- Obtain two IVs, cardiac monitoring, glucose, CBC, CMP with AST/ALT/alkaline phosphatase, fractionated bilirubin, PT/INR, lipase, urinalysis, pregnancy test when relevant, and type and screen.
- Glucose <60 mg/dL or altered patient unable to take oral glucose → dextrose 25 g IV (50 mL D50W; use 100 mL D10W when peripheral extravasation risk is a concern), because immediate substrate prevents hypoglycemic brain injury.
- Hypotension or sepsis with suspected cholangitis → crystalloid 1–2 L IV initially (balanced crystalloid preferred), because restoring preload supports perfusion while source control is arranged.
- Persistent MAP <65 mmHg after fluids → norepinephrine 0.05–0.1 mcg/kg/min IV infusion, titrate, because α-mediated vasoconstriction restores organ perfusion; use a reliable dosing reference/institutional protocol for titration.
- Suspected acute bacterial cholangitis or biliary sepsis → ceftriaxone 2 g IV plus metronidazole 500 mg IV, because this covers enteric gram-negative organisms and anaerobes pending drainage. For severe illness, healthcare-associated infection, or local resistance concerns, use piperacillin-tazobactam 4.5 g IV instead; verify renal adjustment and institutional antibiogram.
- Suspected acetaminophen toxicity or acute liver failure with possible acetaminophen exposure → N-acetylcysteine 150 mg/kg IV over 1 hour, then 50 mg/kg over 4 hours, then 100 mg/kg over 16 hours, because it restores hepatic glutathione and may benefit non-acetaminophen acute liver failure; confirm protocol because dosing regimens vary.
- Avoid empiric vitamin K unless deficiency/warfarin effect is plausible; INR elevation from hepatic failure will not reliably correct with vitamin K.
Definitive Care & Disposition
How This One Kills
- Carotenemia — yellow skin with spared sclera and normal bilirubin; confusing it with jaundice triggers unnecessary invasive workup.
- Hemolysis — indirect bilirubin, high LDH/reticulocytes, low haptoglobin; confusing it with obstruction delays treatment of anemia or hemolytic crisis.
- Drug-induced liver injury — temporal medication/supplement exposure with variable biochemical pattern; missing it perpetuates the offending agent.
- Acute viral hepatitis — markedly elevated aminotransferases often with prodrome; confusing it with cholangitis can delay needed drainage, while assuming it is benign can miss liver failure.

- Draw the bilirubin pathway and classify five practice cases as prehepatic, hepatocellular, or cholestatic.
- Interpret three liver panels using the R-ratio: `(ALT/ULN) ÷ (ALP/ULN)`.
