A 72-year-old man arrives with a sudden, explosive headache and vomit on his shirt, his wife gripping a medication list that trembles in her hand. He is somnolent but opens his eyes to voice; his right arm drifts toward the stretcher rail. The CT scanner is ready, but the medication list shows a blood thinner and the laboratory result that will determine the antidote is still pending.

When to Think of It

Enter the pathway for life-threatening bleeding, critical-site bleeding, urgent surgery, or serious hemorrhage with shock in a patient taking warfarin, unfractionated heparin, LMWH, dabigatran, apixaban, rivaroxaban, edoxaban, or betrixaban. Ask for the last dose, indication, renal function, adherence, and co-medications; bring the medication bottles or pharmacy record if possible.

Sick or Not Sick

The pivotal call is major/critical-site bleeding versus nonmajor bleeding. Intracranial hemorrhage, airway bleeding, hemodynamic instability, expanding hematoma, or bleeding requiring transfusion/procedure warrants immediate reversal while testing proceeds.

The First Fifteen Minutes

  • All major bleeds: Two large-bore IVs, type and crossmatch, CBC, PT/INR, aPTT, fibrinogen, creatinine, anti-Xa or thrombin time when available, and immediate source-control imaging. Activate a massive transfusion pathway if shock is present.
  • Warfarin with major bleeding or INR ≥2: Give vitamin K 10 mg IV slowly plus 4-factor PCC 25–50 units/kg IV based on INR (maximum 2,500–5,000 units; follow product protocol), because PCC rapidly replaces factors while vitamin K sustains synthesis. Check the local PCC dosing table.
  • Dabigatran with major bleeding: Give idarucizumab 5 g IV as two consecutive 2.5-g doses, because it binds dabigatran immediately. If unavailable, use 4-factor PCC 50 units/kg IV with hematology/toxicology input; dialysis may help because dabigatran is dialyzable.
  • Apixaban, rivaroxaban, or edoxaban with life-threatening bleeding: If available, give andexanet alfa—low dose 400 mg IV bolus then 4 mg/min infusion for 120 minutes, or high dose 800 mg bolus then 8 mg/min for 120 minutes according to agent, dose, and timing. If unavailable, give 4-factor PCC 50 units/kg IV; confirm institutional protocol because practice and availability vary.
  • Unfractionated heparin: Stop heparin; if bleeding is major or urgent reversal is needed, give protamine 1 mg IV per 100 units of heparin received in the preceding 2–3 hours, maximum 50 mg, slowly, because it neutralizes heparin. Recalculate for the actual recent infusion.
  • Enoxaparin or another LMWH: If a major bleed occurred within 8 hours, give protamine 1 mg IV per 1 mg enoxaparin; if 8–12 hours ago, 0.5 mg per 1 mg. Reversal is incomplete; obtain expert guidance.
  • Shock: Give balanced crystalloid in small reassessed boluses and blood products early; avoid allowing resuscitation to delay antidote or source control.

Definitive Care & Disposition

Reverse the drug while pursuing source control: emergent neurosurgical consultation for ICH, endoscopy or interventional radiology for GI bleeding, operative control for compressible or expanding hemorrhage, and airway intervention when needed. Repeat hemoglobin, INR/aPTT, fibrinogen, neurologic examination, and imaging according to the bleeding site. All critical-site bleeds require ICU-level care; resume anticoagulation later only after bleeding control and a documented thrombotic-risk discussion.

How This One Kills

The lethal error is waiting for a “confirmatory” drug level or INR before reversing a deteriorating intracranial bleed—especially when the medication history already identifies the anticoagulant.
The Differential — What Else Looks Like This
  • Thrombotic stroke — focal deficit with vessel occlusion rather than hemorrhage; confusing the two can delay reperfusion or cause catastrophic bleeding.
  • Subdural empyema or meningitis — headache and altered mental status with infectious features; anticoagulant reversal alone delays antibiotics and drainage.
  • Spontaneous retroperitoneal bleed — flank pain, femoral neuropathy, falling hemoglobin, and shock with little external blood; missing it leads to delayed embolization or surgery.
  • Traumatic hemorrhage — apparently minor trauma may conceal expanding intracranial or abdominal bleeding; falsely reassuring examination delays imaging.

The Second-Day Story

Older adults may present with isolated confusion, weakness, a fall, anemia, or hypotension rather than obvious bleeding. A partially treated patient may have a near-normal INR despite clinically important factor-Xa inhibition, while renal failure can prolong dabigatran effect long after the reported last dose. Treat the clinical bleeding syndrome and medication exposure—not a single reassuring laboratory value.

He is protecting his airway but is drowsy, hypertensive, and has right-sided weakness; there is no external bleeding. CT shows an acute intracerebral hemorrhage, and we are sending CBC, INR, fibrinogen, type and crossmatch, and renal function.
