High-Pressure Injection Injuries
These injuries are easy to dismiss and catastrophic to miss. The visible skin wound may be trivial while the true injury is chemical, infectious, and ischemic destruction of the hand.
A 46-year-old mechanic holds his gloved hand close to his chest, the skin over his index finger barely marked except for a tiny gray puncture and a crescent of swelling that seems too small for the amount of pain on his face. The finger is dusky, tense, and strangely numb in spots, while the pump room smell of paint thinner still clings to his sleeve. He keeps insisting it’s “just a pinhole” and asks if he can finish the shift. The hand keeps getting tighter while the clock keeps moving, and the next move is still on the table.

When to Think of It
Any history of paint, grease, oil, solvent, hydraulic fluid, or air/grease gun injection into a finger, hand, or other extremity — especially a tiny puncture with disproportionate pain, swelling, paresthesia, pallor, or stiffness. Treat this as a surgical emergency even if the patient looks “too well.”
Sick or Not Sick
The single call that matters most: is there a high-pressure injection into the hand/extremity, especially with organic/solvent material or vascular compromise? If yes, the patient needs emergent hand/orthopedic surgery involvement now; do not let a normal x-ray or small wound reassure you.
The First Fifteen Minutes
NPO, remove rings/jewelry, elevate the limb, and splint → reduces further pressure and vascular compromise.
IV analgesia: fentanyl 25–50 mcg IV q5–10 min PRN or hydromorphone 0.5–1 mg IV q2–3h PRN → pain control so exam and transport are possible.
Tetanus update: Tdap 0.5 mL IM if status unknown or not up to date; add TIG per wound guidelines if indicated → prevents tetanus in a contaminated puncture.
Broad-spectrum antibiotics if contamination is suspected: e.g., cefazolin 2 g IV plus an agent for gram-negatives/anaerobes depending on material and local practice; if farm/soil contamination, consider piperacillin-tazobactam 4.5 g IV now → early bacterial coverage for deep inoculation (regimens vary; check local hand surgery guidance).
Urgent hand surgery/orthopedics consultation immediately → early operative debridement is the definitive limb-saving step.
Avoid field-style “squeeze it out,” incision, or local repair → prevents spread of injected material.
If severe pain, pallor, progressive neurovascular compromise, or concern for compartment syndrome, escalate emergently → ischemia can progress quickly and requires operative decompression.
Definitive Care & Disposition
Definitive care is operative exploration, irrigation, and debridement, often repeated, with removal of injected material and any devitalized tissue. Organic solvents, paint, and caustic substances worsen tissue necrosis and dramatically increase amputation risk. Admit under the hand surgeon; many need OR within hours, not “next day clinic.” Serial neurovascular exams matter because swelling and vascular loss evolve.
How This One Kills
Calling it a “small puncture wound” and sending the patient home. The failure mode is delayed debridement while chemical injury and compartment pressure silently destroy tendons, nerves, vessels, and muscle.
The Differential — What Else Looks Like This
Simple puncture wound — tiny surface injury but without disproportionate pain, tense swelling, or injection history; confusing it delays surgical debridement.
Cellulitis — erythema and pain predominate without injection history or high-pressure mechanism; missing injection injury means source control never happens.
Compartment syndrome — severe pain with passive stretch and tense compartments can overlap; confusing the two delays the mechanism-specific debridement/exploration.
Flexor tenosynovitis — Kanavel signs point to tendon sheath infection rather than injected material; wrong diagnosis can miss the chemical necrosis driving tissue loss.
The Second-Day Story
Older patients, intoxicated patients, and workers who self-treat may underreport the mechanism, presenting hours later with only a tiny wound and a “not that bad” story. Some injectates produce minimal early skin change but deep vascular thrombosis and inflammatory destruction continue under the surface. If the pain, swelling, or numbness is out of proportion to the wound, assume the worst until hand surgery proves otherwise.

Sick or Not Sick
The key fork: is the part potentially salvageable and is the patient a replantation candidate? Clean-cut, cold ischemia within acceptable limits, and thumb/multiple-digit injuries favor urgent microsurgery evaluation; crush/avulsion, prolonged warm ischemia, severe contamination, or unstable patients lower the chance of salvage.
The First Fifteen Minutes
Direct pressure/tourniquet for uncontrolled bleeding → hemorrhage control first; a brief tourniquet is acceptable for life- and limb-threatening bleeding while definitive control is arranged.
IV analgesia: fentanyl 25–50 mcg IV q5–10 min PRN or hydromorphone 0.5–1 mg IV q2–3h PRN → permits exam and wound care.
Tetanus prophylaxis: Tdap 0.5 mL IM if not up to date; TIG if indicated → contaminated traumatic wounds need protection.

Definitive care is operative replantation in selected candidates or revision amputation with soft tissue coverage and hemostasis. Transfer to a microsurgical center if replantation is possible and not available locally. Admit for pain control, wound care, antibiotics as indicated, and serial neurovascular monitoring.
How This One Kills
Putting the amputated part directly on ice, in water, or dry on a counter. The failure mode is avoidable tissue death from freezing or desiccation, eliminating salvage options before the surgeon ever sees it.
The Atypical Presentation
Crush injuries and avulsions often look “messier” than the textbook clean cut, but they are less salvageable because the vascular intima and soft tissue are shredded beyond repair. Elderly patients on anticoagulants may appear to have a minor injury while bleeding is actually substantial. Pediatric digit amputations deserve extra urgency because replantation success and functional recovery are often better than in adults.

Beta-blockers blunt tremor and tachycardia, and renal failure prolongs sulfonylurea action. If the glucose rebounds after dextrose but then falls again, assume ongoing insulin effect until proven otherwise.

She needs prolonged observation and likely admission because the danger is not the first low but the next one after the bolus wears off. Her disposition depends on whether the glucose remains stable off infusion and whether the ingestion was accidental or intentional.

Mechanism Pearl of the Day: Several of today’s emergencies are dangerous because the visible problem underestimates the hidden one: a tiny wound with deep tissue destruction, a missing fingertip with ischemic tissue viability at stake, and a “simple intoxication” that is actually metabolite-driven systemic injury. In each case, the mechanism tells you the real severity before the surface does.
Key Medications
Dextrose 25 g IV bolus: D50W 50 mL IV or D10W 250 mL IV
Glucagon 1 mg IM/SC/IV once if no IV access
Octreotide 50–100 mcg SC or IV q6h for sulfonylurea/meglitinide toxicity
D10W infusion 100–200 mL/hr or titrate to glucose
Thiamine 100 mg IV
Lorazepam 2–4 mg IV for seizure
Activated charcoal 1 g/kg PO/NG early after ingestion if appropriate; check poison center
Pediatric dosing differs substantially for dextrose and octreotide; check pediatric reference
High-Yield Pearls
A single “normal” glucose after dextrose does not end the case if sulfonylurea exposure is possible.
Octreotide is the key antidote for recurrent sulfonylurea hypoglycemia.
Hypoglycemia can masquerade as stroke, intoxication, or delirium; fingerstick glucose is the fastest discriminator.
The Mimics
Insulin overdose — insulin causes hypoglycemia without driving endogenous insulin secretion; confusing it with sulfonylurea toxicity leads to missing octreotide.
Sepsis — altered mental status can mimic neuroglycopenia, but fingerstick glucose is the discriminator; missing the glucose check delays lifesaving treatment.
Alcohol intoxication — slurred speech and ataxia overlap, but hypoglycemia may coexist and should be checked in every intoxicated patient; confusing them can be fatal.
Stroke — focal deficits can occur with severe hypoglycemia; treating as stroke alone delays glucose correction and can worsen outcomes.

Empiric coverage for nosocomial / healthcare-associated pneumonia, neutropenic fever, complicated UTI with Pseudomonas risk, and select sepsis regimens.

ED Dose
2 g IV q8h for severe infection or neutropenic fever. 1–2 g IV q12h for less severe infections.
Renal Adjustment
CrCl 30–60: 2 g q12h. CrCl 11–29: 2 g q24h. CrCl ≤ 10: 1 g q24h. Hemodialysis: 1 g q24h after dialysis.
Renal function. Cefepime-induced neurotoxicity (encephalopathy, non-convulsive status) particularly when underdosed for renal function — have a low threshold for EEG in altered patients on cefepime.
ED Pearl
If a patient on cefepime becomes unexplainedly altered, suspect cefepime neurotoxicity before chasing other diagnoses — it resolves with drug discontinuation but is frequently missed.

Diffuse concave ST elevation with PR depression is pericarditis, but the ED job is to separate it from STEMI and myocarditis.
The Tracing
A 31-year-old man presents with sharp pleuritic chest pain that improves when leaning forward. The ECG shows diffuse concave ST elevation in I, II, aVL, aVF, and V2 through V6 with PR depression in multiple leads and reciprocal PR elevation in aVR. There is no focal reciprocal ST depression except in aVR and V1.

Pearls
PR depression is the high-yield clue that points toward pericardial inflammation.
Pericarditis should be a clinical diagnosis supported by ECG, not an ECG-only label.
Myopericarditis can have troponin elevation; the disposition changes when myocardium is involved.
Pitfalls
Do not call focal territorial ST elevation pericarditis just because the patient is young.
Do not ignore reciprocal ST depression outside aVR/V1; that should pull you back toward STEMI.
Do not miss tamponade red flags: hypotension, JVD, dyspnea, muffled heart sounds, large effusion, or pulsus paradoxus.
At the Bedside
Evaluate for STEMI mimics and high-risk pericarditis features, check troponin/inflammatory markers as appropriate, obtain echo if concern for effusion/myocarditis, and treat/dispose per risk profile.
