Retinal Detachment and Defects
Retinal detachment is a time-sensitive cause of irreversible vision loss. The resident’s job is to recognize it early, protect the eye, and get the right ophthalmology response without delay.
A 62-year-old man leans into triage with one hand over his right eye, describing a curtain of darkness that slid up from the lower vision while he was reading the morning paper. He’s calm but uneasy, pointing to a swarm of new black specks and a flash that cut across the side of his sight an hour ago. The room is bright, his exam otherwise ordinary, but his words keep circling the same unnerving theme: something is missing from the picture. He’s waiting to hear whether this is a problem that can safely wait.

When to Think of It
Monocular flashes, floaters, a curtain or field cut, or sudden painless loss of peripheral vision. Think of it when the patient says vision is “missing,” “shadowed,” or “drawn closed,” especially after trauma, high myopia, prior surgery, or a history of detachment in the other eye.
Sick or Not Sick
The single fork is macula-on vs macula-off. Macula-on detachment is vision-sparing only until it isn’t; macula-off is still urgent, but the immediate pressure is greatest to save the macula before central vision is lost.
The First Fifteen Minutes
No medication fixes this.
If globe rupture is possible: place a rigid eye shield now, keep NPO, give antiemetic if vomiting threatens eye pressure — ondansetron 4–8 mg IV because retching raises intraocular pressure.
If pain/nausea present: ondansetron 4–8 mg IV and acetaminophen 650–1000 mg PO or IV as needed, because comfort helps prevent Valsalva.
Do not patch the eye, press on it, or check a tonometer if open globe is on the table.
Definitive Care & Disposition
Urgent ophthalmology evaluation is required, with timing driven by macular status and detachment extent. Most patients need operative repair (pneumatic retinopexy, scleral buckle, or vitrectomy) and should be kept NPO with eye protection until seen. Disposition is typically urgent specialty-directed management, often same-day or next-day depending on macula status and local ophthalmology guidance.
How This One Kills
The classic failure is dismissing a “floater” complaint as benign until the detachment reaches the macula. Once central vision is lost, recovery is much worse.
The Differential — What Else Looks Like This
Posterior vitreous detachment — flashes/floaters without a true field defect; confusing it delays the retinal tear check.
Central retinal artery occlusion — sudden painless monocular vision loss, but usually profound and not curtain-like; confusing it with detachment misses stroke-style escalation.
Ocular migraine — transient shimmering visual symptoms that resolve; confusing it with detachment delays eye exam.
Vitreous hemorrhage — floaters and haze without a peripheral field curtain; confusing it for detachment can miss a tear.
The Second-Day Story
Older adults, diabetics, and post-op patients may describe only blurred vision, a gray veil, or “just not seeing right” rather than a dramatic curtain. In partial detachments the exam can look deceptively normal unless you press for field loss, flashes, and floaters; if the story sounds retinal, treat it as retinal until ophthalmology rules it out.

Eye Foreign Bodies
Missed penetrating eye injuries can lead to endophthalmitis, vision loss, and surgical catastrophe. Even a tiny foreign body can hide a high-risk mechanism.
A 29-year-old machinist arrives blinking hard, one eye half closed, a tiny silver fleck glinting on his lower lid in the fluorescent light. He says it started the instant a grinder sparked across the shop floor, and now every blink feels like sandpaper. He’s wearing his work boots, still smelling faintly of metal and sweat, but he’s more worried about the sensation than the object itself. The question is whether this is a superficial nuisance or a dangerous penetrating injury.

Why It Matters

When to Think of It
Metal-on-metal grinding, hammering, mowing, drilling, or any high-velocity projectile exposure; eye pain, tearing, photophobia, foreign body sensation, and blepharospasm. Think penetration if there is decreased vision, irregular pupil, hyphema, Seidel-positive leak, or a misshapen globe.
Sick or Not Sick
The single fork is superficial corneal/conjunctival foreign body vs penetrating/open-globe injury. If penetration is possible, stop and protect the eye; if clearly superficial, remove gently and confirm no retained rust ring or corneal defect.
The First Fifteen Minutes
Topical anesthetic for exam only: proparacaine 0.5% 1–2 drops to facilitate inspection, because it enables a careful exam; do not prescribe for home use.
If superficial and visible: irrigate with normal saline; if needed, remove with a moist cotton swab or under slit lamp with a spud/needle by trained clinician, because direct extraction relieves symptoms and prevents epithelial damage.
If concern for open globe: place rigid eye shield immediately, keep NPO, give ondansetron 4–8 mg IV if nauseated, because vomiting raises IOP and worsens extrusion.

Suspected penetrating injuries require emergent ophthalmology, shield, NPO, and likely CT orbit for retained metallic foreign body if the globe is closed enough for imaging. Do not remove an embedded object in the ED if it appears intraocular.
How This One Kills
The classic error is treating a high-velocity metal strike like a simple corneal scratch and then irrigating or manipulating an open globe. That can convert a contained injury into permanent vision loss.
The Atypical Presentation
The eye can look deceptively quiet if the object is tiny, retained under the lid, or has already dislodged. Contact lens wearers and patients with reduced sensation may underreport pain; any high-velocity mechanism with persistent unilateral symptoms deserves a fluorescein exam and lid eversion, with a low threshold for imaging and ophthalmology if the story is concerning.

The First Fifteen Minutes

Head of bed elevated 30–45°, because blood settles inferiorly and clears the visual axis.
Acetaminophen 650–1000 mg PO/IV for pain, because NSAIDs can worsen bleeding.
Ondansetron 4–8 mg IV if nauseated, because vomiting raises venous and intraocular pressure.

How This One Kills
The mistake that blinds is missing the pressure rise or rebleed window and sending the patient home with NSAIDs and no follow-up. Rebleeding often occurs days later, when the eye “looks better.”
The Atypical Presentation
Small hyphemas can be nearly painless, especially in children and athletes who minimize symptoms. In sickle cell disease or trait, even a modest hyphema can produce outsized pressure problems, so the exam has to include IOP once globe rupture is excluded and follow-up cannot be casual.

Back to our 16-year-old basketball player with the visible blood level: this is a traumatic hyphema until proven otherwise. He is shielded, kept upright, given acetaminophen and antiemetic support as needed, and screened for pressure elevation only after open globe is excluded. If the hyphema is small and the pressure is normal, he may go home with strict restrictions and next-day ophthalmology; if pressure is high, the hyphema is large, or follow-up is unreliable, he needs admission.

Uncomplicated aspiration pneumonia is generally treated with standard community-acquired pneumonia regimens; routine isolated anaerobic coverage is not required. Ampicillin-sulbactam is also reasonable, especially if aspiration risk is high, while metronidazole or vancomycin alone is inadequate.

Antimicrobial of the Day
Penicillin G (aqueous)
Natural penicillin (IV)
Indication
Neurosyphilis and syphilis in pregnancy, Group A/B streptococcal infections, intrapartum GBS prophylaxis, susceptible meningococcal disease, susceptible streptococcal endocarditis, and clostridial infections.
What’s your dose? — reveal dosing & cautions
ED Dose
Aqueous crystalline: 2–4 million units IV q4h. Neurosyphilis: 18–24 million units/day (continuous infusion or 3–4 MU q4h) x10–14 days. GBS prophylaxis: 5 MU IV load, then 2.5–3 MU IV q4h until delivery.
Renal Adjustment
Reduce dose in severe renal impairment; monitor for neurotoxicity from accumulation.
Contraindications
Penicillin hypersensitivity.
Interactions
Probenecid increases and prolongs levels.
Monitoring
Anaphylaxis/allergy, sodium and potassium load with high-dose therapy, neurotoxicity (seizures) at high doses or in renal failure.
ED Pearl
Aqueous IV penicillin G is NOT interchangeable with benzathine penicillin G (IM, for early syphilis) — and benzathine must never be given IV (can be fatal). Neurosyphilis requires the aqueous IV formulation.
RSI paralysis, ventilator synchrony paralysis in selected intubated patients, and alternative to succinylcholine when contraindications exist.

ED Dose
RSI: 1.0–1.2 mg/kg IV using actual body weight in many ED protocols. Lower doses 0.6 mg/kg have slower onset and longer time to ideal intubating conditions.
Renal Adjustment
No formal single-dose adjustment, but duration may be prolonged in hepatic/renal dysfunction.
Contraindications
Hypersensitivity to rocuronium/aminosteroid NMBAs. Cannot intubate/cannot ventilate risk is a clinical contraindication to paralysis without rescue plan.
Continuous sedation/analgesia while paralyzed, train-of-four if ongoing, ventilator/airway security.
ED Pearl
Roc lasts much longer than etomidate or ketamine — paralysis without analgesia/sedation is a never-event you prevent with orders before intubation.

Rhythm
Torsades de Pointes
Polymorphic VT in the setting of prolonged QT — treat the rhythm and remove the trigger.
The Tracing
A 44-year-old woman treated with methadone and ondansetron becomes lightheaded, then briefly loses consciousness. The monitor shows a rapid wide-complex rhythm whose QRS axis appears to twist above and below the baseline. Between bursts, the ECG shows a markedly prolonged QT interval with a long pause before the next run. She wakes up pale and diaphoretic as the rhythm self-terminates.
Polymorphic ventricular tachycardia with QRS amplitude and axis waxing and waning around the baseline
Occurs in the setting of prolonged QT or prominent pause-dependent QT prolongation
Often initiated by a premature ventricular beat after a long-short sequence
May be self-terminating or deteriorate into ventricular fibrillation
Baseline ECG often shows QTc prolongation, bradycardia, electrolyte abnormality, or offending drugs
Pearls
Magnesium is first-line even if the serum magnesium level is normal.
Pause-dependent torsades improves by increasing the heart rate: overdrive pacing or isoproterenol can be the bridge when recurrent despite magnesium.
Think medication stack: methadone, antipsychotics, macrolides, fluoroquinolones, antiemetics, and electrolyte depletion often combine.
Pitfalls
Do not treat torsades like stable monomorphic VT with QT-prolonging antiarrhythmics; that can worsen the substrate.
Polymorphic VT with a normal QT is often ischemic until proven otherwise and needs a different pathway.
QTc measurement after the event matters; the monitor strip alone tells you the rhythm, not the cause.
At the Bedside
If pulseless or unstable, defibrillate. If perfusing, give magnesium sulfate 2 g IV, correct potassium/magnesium, stop QT-prolonging drugs, and consider overdrive pacing or isoproterenol for recurrent pause-dependent episodes.
